New Clients using Health Insurance or Self Paying may download and complete the  three forms below and bring to our first session to save time.   If you are unable to download. print and fill our these forms, you may fill them out at our first appointment.  

Clients using an Employee Assistance Programs (EAP) would complete the Client Registration Form and either the EAP Specific  Statement of Understanding, which I will email  you,. or in some cases the one on here, 

In our first conversation if other forms, like the Depression Screening, or Relationship Questionnaire, might be helpful, I will forward those to you.
